Position management

Global rule for all modes: evaluate the active position's ladder top to bottom on each closed candle of ITS OWN timeframe (5m for scalps, 15m for everything else); first match acts. Priority numbers are authoritative. A position always uses the ladder of the mode that opened it.

Hard stop hit -> close 100%.
News window (FOMC/CPI/NFP within 15 min) -> close 100%.
Breakout failure before TP1 (close back beyond the broken level) -> close 100%.
